Power-assisted liposuction (PAL):
Advantages and clinical benefits in aesthetic surgery
Why is power-assisted liposuction (PAL) considered one of the established liposuction techniques?
Power-assisted liposuction (PAL) can help reduce the manual effort required and symptoms of fatigue during fat aspiration compared with the standard procedure. The power-assisted (or vibration-assisted) cannula guidance enables surgeons to work with precision, particularly during longer procedures, high-volume liposuction and revisions involving complex tissue structures¹ ².
Partly due to these ergonomic advantages, PAL is now considered one of the established techniques in modern liposuction, alongside suction-assisted liposuction (SAL), water-assisted liposuction (WAL), ultrasound-assisted liposuction (UAL) and laser-assisted liposuction (LAL).
In the following, we use the term ‘PAL’ as an abbreviation for power-assisted liposuction. This trademarked term has become established in everyday clinical practice as the common designation for power-assisted liposuction procedures.
Liposuction procedures at a glance
Procedure | Characteristics | Special attributes |
|---|---|---|
SAL | Purely manual cannula guidance during suction-assisted liposuction | A proven standard procedure for many routine procedures |
WAL | Water-assisted liposuction | Fat aspiration that is gentler on the tissue than the standard procedure |
UAL | Ultrasound-assisted liposuction | Support with fat emulsification |
LAL | Laser-assisted liposuction | A combination of liposuction and skin tightening |
PAL | Power-assisted/vibration-assisted liposuction | Reduction in the amount of manual effort required, easier handling of larger volumes and more complex tissue structures, whilst minimising signs of fatigue in the surgeon |
When do experienced surgeons opt for PAL?
Experienced surgeons often opt for PAL when precision, tissue preservation and ergonomic working are key priorities. The vibration-assisted cannula guidance facilitates controlled contouring even in challenging treatment areas, reduces the manual effort required during longer procedures and contributes to consistent, reproducible treatment outcomes¹ ².
Benefits of PAL (power-assisted liposuction) at a glance

Less physical strain – even during lengthy procedures

Precise fat aspiration

Efficient integration into daily surgical practice
How does power-assisted liposuction work?
Power-assisted liposuction aids fat aspiration through motor-generated high-frequency vibrations of the cannula. Unlike purely manual liposuction, the cannula is no longer guided exclusively by the surgeon; instead, the surgeon’s movements are mechanically assisted by the vibrations. This facilitates controlled, uniform and gentler fat aspiration whilst reducing the manual effort required¹. These advantages are particularly evident in procedures that place high demands on precision and standardised procedures – such as the surgical treatment of lipoedema.
PAL in lipoedema surgery
Surgical treatment of lipoedema places particular demands on the surgeon. Compared with many conventional cosmetic liposuction procedures, it is often necessary to
- treat larger areas
- cope with longer operating times
- and perform multiple procedures
Furthermore, revision surgery in particular can be complicated by fibrotic tissue structures.
Under these conditions, precise cannula guidance, ergonomic working practices and standardised surgical procedures become even more important.
This is precisely where power-assisted liposuction demonstrates its particular benefits. It does not replace the surgeon’s experience, but supports them in working precisely and reproducibly, even under demanding conditions. The vibration-assisted cannula movement facilitates controlled fat aspiration – even when the duration of the procedure, tissue characteristics and the extent of the operation place particular demands on the surgeon.

Controlled liposuction for challenging areas and revision procedures
High-volume liposuction and revision procedures on previously operated-on or fibrotic tissue require uniform and controlled fat aspiration. Power-assisted cannula guidance facilitates precise manipulation of challenging tissue structures.

Ergonomic working throughout the entire operation
Longer procedures place considerable strain on the hand, arm and shoulder. Reducing the amount of manual effort required promotes ergonomic working and helps maintain concentration even over several hours.

Reproducible results and a high level of patient safety
Particularly in complex operations, such as lipoedema treatment, it is essential to work in a controlled and precise manner throughout the entire procedure. Standardised procedures, safe cannula guidance and ergonomic working conditions create the key conditions for reproducible treatment results.
What is important in a modern PAL system?
A PAL system supports the surgeon throughout the entire procedure. Therefore, it is not just the technical features that determine its usefulness, but above all how well the system integrates into day-to-day operating theatre routines and clinical workflows. In addition to precise cannula guidance, ergonomics, ease of use and efficient workflows now play a crucial role. Particularly when there are several procedures a day or during lengthy operations, these factors can help to establish standardised surgical processes.

Frequently asked questions (FAQs)
When is the use of a PAL system particularly beneficial?
Power-assisted liposuction is generally suitable for all forms of liposuction. It is particularly advantageous in situations where long operating times, large treatment volumes or fibrotic tissue structures place high demands on precision and physical and cognitive ergonomics1. The vibration-assisted cannula movement reduces the manual effort required and facilitates controlled fat aspiration – particularly during demanding or repeated procedures.
What role does the choice of cannula play in power-assisted liposuction?
Not only the PAL system, but also the choice of cannula can significantly influence the treatment outcome. It is the combination of a suitable cannula and modern PAL technology that creates the essential conditions for precise and controlled fat aspiration. Depending on the treatment area, tissue characteristics and surgical objective, different cannulas are used, which vary in terms of diameter, length and geometry, amongst other factors. The choice of cannulae should therefore always be tailored to the specific indication, the desired treatment outcome and the surgeon’s personal preferences.
Why does PAL contribute to reproducible treatment outcomes?
Reproducible outcomes require a precise and controlled approach. The vibration-assisted cannula movement helps the surgeon to perform liposuction evenly, even during longer procedures or when dealing with challenging tissue structures. Combined with standardised surgical procedures, this creates favourable conditions for consistently high treatment quality and reproducible treatment outcomes.
Why is PAL used in revision procedures?
Revision procedures often place greater demands on liposuction, as previously treated tissue frequently exhibits fibrotic changes. The vibration-assisted cannula movement facilitates controlled cannula guidance even when tissue resistance is high, whilst simultaneously reducing the manual effort required. This facilitates precise work and helps surgeons to operate efficiently even under these challenging conditions. Is PAL only suitable for complex liposuction procedures, such as those for lipoedema?
No. Although the advantages of power-assisted liposuction are particularly evident in lengthy procedures, revision surgery or cases involving fibrotic tissue structures, it can be used for all forms of liposuction. Many surgeons also use PAL specifically for demanding body-contouring treatments in order to benefit from the possibilities offered by precise fat aspiration.
